  1. Have your questions ready in another program, such as a word processor or Notepad
    1. Organize the questions into the following chunks:
      1. Key outcome variables
        Such as "I look forward to coming to work" or "Morale in my work group is high"
      2. Questions about the company in general
        Such as "This company supports people having a balanced work and home life"
      3. Questions about senior leadership
        Such as "By their actions and decisions, our senior leaders demonstrate their knowledge of our customers and our marketplace."
      4. Questions about their department
        Such as "In my department we review each project to learn how to improve our project management."
      5. Questions about their manager or supervisor
        Such as "My manager is careful to get accurate information on my performance before my formal review."
      6. Questions about their work group
        Such as "People in my immediate work group are supportive and cooperative."
      7. Questions about their own work experience
        Such as "I have so much work stress that it is causing problems in my personal life or family relationships."
      8. Demographics
        Such as their department, their job type, their gender, their tenure with the company, and so on.
        Save the demographics until the end so that the respondent knows what information they've given before they risk identifying themselves. It's a good idea to preface the demographics with a comment such as "These last few questions will help us to better understand the information you've already given. If you feel at all uncomfortable answering any of these questions, feel free to skip some." The comfort of the respondent is more important than having a complete data set.
  2. Sign into your SurveyMonkey account, set up a new survey (name, template, etc.) and start adding questions. Cut 'n Paste from the text in your word processor.
  3. Be careful about the question type.
    If you want to use a scaled response, such as 1 to 7, use the special option under the Matrix structure.
  4. Avoid using rank order of any kind. The data is inherently difficult to process and almost always misleading.
